REGISTRATION

Registration is mandatory to all delegates to participate in the 17th Asian Conference on Clinical Pharmacy. We encourage each delegate to register using the online registration system. When delegates are registered by the third party, it is mandatory to provide details of personal information during the registration process.

It is NOT possible to do a collective registration using the online registration system, please refer to Collective Registration Form. Payment can be collective; please mention all participant names. Proof of payment for collective registration should be put into one file and submitted online using the Payment Proof.

Please note that Student Category refers to Bachelor or Undergraduate Student only and should submit a valid proof of current student status to our e-mail accp-secretariat@ff.unair.ac.id.

Notes :

  1. Conference fee includes conference kit, snacks, lunch, exhibition, and access to all symposiums, plenary, keynote sessions, oral and poster presentations.
  2. Pre-conference workshop fee includes workshop kit, snack and lunch.
  3. Student category refers to Bachelor or Undergraduate student and should submit a valid proof of current student status to our e-mail accp-secretariat@ff.unair.ac.id; Student has access to all conference programs, exhibition, and gala dinner.
  4. Accompanying person refers to a spouse, friend or adult child (18 years old and older) who will accompany the delegate/participant at the conference, BUT they do not attend the scientific sessions. Accompanying person has limited access to exhibition and gala dinner only.
  5. All meals are pork-free, and vegetarian meals can be provided during the conference. Please send your specific dietary requirements or food request to our e-mail accp-secretariat@ff.unair.ac.id before July 14, 2017.
  6. No additional fee for Gala Dinner and University & Hospital Visit, but you have to register if you want to join the Visit.

Important Notes :

  1. The beneficiary account owner (recepient) name for the bank account has to be written as ACCP Indonesia 2017, otherwise bank transaction will be rejected. Also please mention the participant full name(s) in the bank transaction document.
  2. It is mandatory to provide full address of beneficiary (including country) and payment details for Telegraphic Transfer requests. Requests received with incomplete details will not be processed.
  3. PayPal, card provider or inter-bank transfer may charge a transaction fee and it should be paid in addition to the total payment.
  4. All participants are required to submit SWIFT copy of their payment using the Payment Proof available at the conference website. SWIFT copies provide the name and code of the originating bank and several preset codes that provide a message to the receiving bank to confirm your money transfer application.
  5. Your payment status will be confirmed after the payment has been verified. Your payment receipt will be sent to your e-mail, and name of sender will be written in the official receipts.

CANCELLATION AND REFUND POLICY

Cancellation must be advised in writing to the Organizing Committee. Registration cancellation received by June 27, 2017 will get a refund of registration fees less an administrative charge of USD 80 or IDR 750,000. Refunds for cancellation received after June 27, 2017 will be at the discretion of the Organizing Committee. Eligible refunds will be issued two weeks after the conclusion of the conference. Registrations are transferable to a colleague at any time prior to the event provided the Organizing Committee is advised in writing.

OFFICIAL INVITATION LETTER FOR VISA

You can apply for an official invitation letter for visa purpose during the online registration process. Please note that this letter is not a commitment on the part of the organisers to provide any financial support. The 17th ACCP Organizing Committee cannot be held responsible for registrations that need to be cancelled after a visa declination. Please note that terms of cancellation are applicable in all circumstances, also if a visa is officially refused to the participant.
